Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the preponderance of evidence, there is sufficient evidence to support that the operation did not notify the parents when there was an outbreak of lice.”
“The toddler classroom had one caregiver to 10 children ni care. The specified age group (SAG) was 20 months. The director made changes and moved the children to a classroom that had less children in care.”
“Three caregivers' personnel file had the educational documents translated to English; however, they lacked the U.S. education equivalent on file.”
“There was one child's record that lacked the updated immunization record for the age of the child.”
“Two children's admission forms used were outdated. The admission forms do not capture the required minimum standards changes of March 2023.”
“There were 4 children's records that lacked the updated immunization record for the age of the child.”
“One child's admission record that met the requirement lacked a vision and hearing screening in the file.”
“There were 2 classrooms which had the cots arranged with no space for the caregiver and the children to walk in between. This was corrected during the inspection when the director and caregivers moved the cots.”

“Based on the preponderance of evidence gathered, there is sufficient evidence to determine a child was left unsupervised on the playground which was not reported to Child Care Regulation.”
