Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A child was able to exit the restroom and enter an unattended classroom resulting in being unattended for 8 minutes.”
“A caregiver demonstrated poor judgement when they failed to verify a new pick-up person they had not previously met resulting in the caregiver releasing the wrong child to the pick-up person.”
“One child's allergy medication had expired on 2/2025.”
“Caregivers failed to conduct a name-to-face headcount when entering the building resulting in a child being left outside unattended.”
“The operation's parent handbook was missing information regarding the promotion of physical activity involving infants and has not updated the minimum time children will engage in moderate to vigorous play each day.”
“A cabinet under the sink in the school-age room was unlocked containing Clorox wipes, air freshener, Borax activator and cleaning spray bottles. NOTE: Operation replaced the lock on the cabinet and secured the chemicals.”
“2 classrooms did not have a cot chart to identify which child slept on each assigned cot. 2 classrooms had a cot chart that was not current and the child listed on the cot chart did not match the child sleeping on the cot.”
“The electronic safety alarm on bus #1 was not functioning during the inspection.”
“Based on information gathered during the investigation, a child received an injury and continued to show signs of being injured by crying and not being able to put weight on their leg for over an hour. The caregiver failed to relay this information to management.”
“Based on information gathered during the investigation, a child received an injury while in care, was immediately unable to put weight on their leg and continued to be unable to put weight on the leg while outside on the playground including when the parent picked up at the end of the day an hour after the accident occurred.”
