Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The pre-school teacher stated there were 26 children, but when we verified, there were 27 children in care.”
“Through an investigation, it was determined that crafting pom poms were utilized in a group activity that posed a risk of choking.”
“It was determined that prohibited punishment was used when a caregiver aggressively attempted to place a child in a chair and when the child was grabbed around their neck, resulting in scratches to the child's neck.”
“Based on information gathered during the investigation, a child was left unattended on the playground while transitioning to the classroom.”
“1 out 10 classrooms did not have an emergency evacuation diagram posted. This was corrected when management posted the diagram in the classroom.”
“A 16 month old was combined in a room with older children during opening time for at least 10 minutes.”
“It was found that infants were mixed with older children during early morning drop off. The infants were mixed for approximately 1-2 minutes”
“Based on the evidence gathered, a caregiver did not account correctly for all of the children in their care when they transitioned a group of children from one classroom to another. This caregiver is no longer employed at the operation.”
“Based on the evidence gathered, a caregiver did not ensure appropriate supervision of a five-year-old child in their care, resulting in the child being left alone in a classroom for 30 minutes.”
