Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver's training record reviewed was missing annual training for training year 7/19/2024-7/18/2025. Training missing was on the following topics: (1) Preventing and controlling the spread of communicable diseases, including immunizations; and (2) Administering medication.”
“A caregiver caring for a group of children did not have training in pediatric first aid with rescue breathing and choking.”
“Three yogurt packets and a cup of yogurt stored in the refrigerator of an infant room were expired. Note: This was CORRECTED during inspection as the expired items were discarded.”
“A caregiver caring for a group of children completed their pediatric CPR training online.”
“A 5-month-old infant was asleep on a restrictive device (bouncer) for 10 minutes before caregiver was instructed to move the infant to their crib.”
“The carbon monoxide alarm detector, monthly tests, were not documented as required.”
“The required monthly fire extinughiser checks were not documented.”
“The director does not have a current Pediatric First Aid and CPR certification.”
“A child was injured when an employee lacked self-control when redirecting a child while on their cot.”
“In the infant classroom, the space used for shelter in an emergency, was observed to have little space with cleaning equipment, toys and a step ladder. ***corrected at inspection”
“In a hallway bathroom that the children have access to and use, it was observed to have toxic chemicals in a carbinet without locked doors. ***corrected at inspection”
“The fire inspection was observed to be expired and a current up-to-date inspection was not on file.”
“Two contract workers with a private company were observed to be in direct contact with children.”
“One employee's purse and/or handbag was observed sitting on the floor in the classroom which was accessible to children in care. In another classroom, a pair of adult sized scissors and two tape dispensers were observed in an unsecured drawer which was accessible to children. NOTE: Corrections were made at time of inspection.”
