Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“According to the Texas Comptroller website, the business entity's Franchise Tax Account Status is Inactive/Forfeited. The operation is currently providing care to children with an inactive status.”
“During the inspection, a caregiver left the infant room out of ratio when they left the room, leaving one caregiver with six infants. Note: This was corrected at inspection when one infant was moved to another classroom.”
“The staff were found to have up to date CPR training but were lacking the required in-person portion of the course.”
“The use zones around the slides on the playground were less than 6 feet and overlapped with each other.”
“During naptime, a pre-k classroom was left unsupervised when the caregiver in the room dozed off for five minutes.”
“Six out of six staff files reviewed during inspection did not have medication training in the last full year of employment.”
“The infant classroom was observed out of ratio as there were two caregivers observed with 11 infants. Majority of the infants were 0-11 months. The correct ratios is two caregivers to 10 infants. Note: This was corrected at inspection as the oldest infant was moved to another classroom.”
“The operation does not have a current gas inspection.”
“A child with diagnosed food allergy did not have a food emergency plan.”
“A vent was observed with built of of dust.”
“The operation used prohibited punishment when they sent a child to "thinking time", which is a time out, for toileting accidents.”
“Infant bottle was observed not labeled during inspection. This was corrected at inspection.”
“An 18 month old was observed not resting in an age appropriate sleeping equipment. This was corrected at inspection when the child was provided a sleeping mat.”
“Monthly feeding schedule was not available during time of inspection.”
“A child diagnosed food allergy was not posted in the child's classroom. Note: This was corrected at inspection as the Director posted the child's diagnosed food allergy during inspection.”
“A fire inspection was not conducted at the operation within the past 12 months. The last fire inspection was conducted was on 8/25/2020.”
