Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“2 out of 10 children's records missing the name, address, and telephone number of the child s physician or anemergency-care facility.”
“The operation changed directors on 10/3/25, and needed to make notification to Child Care Regulation within 5 days of the changes.”
“An outside music vendor without an approved background check was in ratio with children.”
“A review of the Employees Who Left Employment in 2024 data from January 17, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.”
“During the inspection it was observed that there was an individual present at the operation, with direct access to the children in care, without a clearance from CBCU that they were eligible to be present.”
“During the investigation it was found that a child was left alone without any caregivers supervising them for at least 23 minutes on 11/12/2024. This was cited. Supervision was observed during the inspection to meet compliance.”
“The director first aid training expired 12/13/2020 and the HISD teacher did not have first aid training.”
“There was sufficient evidence that this event occurred.”
“There were 4 staff records that did not have a current CPR/1st training.”
