Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the inspection it was noted an incident report is not given electronically or by paper to parents. A message is sent through Brightwheel, the app used.”
“A bib was being cleaned in the hand washing sink. The caregiver indicated that she washed dirty toys in the hand sink as well. This was corrected at inspection when the bib was removed and a conversation was had with the director on using the hand washing sink for hand washing only.”
“A child was observed sleeping in a restrictive device. This was corrected at inspection when the child was immediately removed and placed in a crib on their back.”
“Two employees with a provisional status were in the same room together. This was corrected at inspection when a caregiver without conditions was added to the room.”
“A child was left unsupervised on the playground for eight minutes.”
“A portion of vinyl trim in the Ones room has fallen off, leaving a hole in the wall.”
“One of six staff files did not have proof of educational requirements.”
“Over the counter medication was expired. This was corrected at inspection when the medication was disposed of.”
“Three of four children's files audited did not have current immunization records.”
“One of four children's files audited did not have a health statement on file.”
“A push cart track did not have a six-foot use zone around it. This was corrected at inspection when the track was removed from the playground.”
“One of two infants not on table food did not have current infant feeding instructions.”
“Six infant bottles were not properly labeled with the child's name. *Bottles were immediately labeled”
“Supervision was compromised when a 13-month-old child was left unsupervised on the playground. The child was later found unharmed by a parent.”
“The operation failed to notify the parents immediately after a child in their care was placed at risk.”
“The caregivers in charge of the infant classroom were negligent when they failed to account for a child who was left unattended on the playground for 32 minutes.”
“Preparedness plan was found missing how the operation will continue to care for children in the new location.”
“Operation policies were missing the time durations, and benefits, of how the operation will be promoting indoor and outdoor physical activity.”
“The caregiver was noted using her personal phone as she was supervising 11 children ages 5-6 year olds.”
“A tylenol bottle was found in the infant refrigerator , the bag did not have the child's name it pertained to. *Director removed medication from the room.”
“There was sufficient evidence to determine the neglectful supervision of a two-year-old child who exited the outdoor gate unattended and was found uninjured by another caregiver.”
“Supervision was compromised when a child was left unattended for approximately 10 minutes.”
“Operation notified CCR on 2/22/22 of the COVID cases experienced in January and did not report to Licensing timely.”
