A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“There were 14 children observed to be in a shared sleeping location with one care giver and three children under 18 months. The director corrected this by removing the three children under 18 months.”
“An infant was put to sleep with a hat on which came off and was under the infant's head. This was corrected at inspection when the loose item was removed from the crib.”
