Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Out of the ten children's records monitored, two were missing the emergency care authorization, one was missing the special care statement, one was missing the healthcare provider statement, and three were missing current and updated immunization records.”
“In the infant classroom, infant equipment currently used was observed to be unsanitary.”
“One out of the 8 records monitored did not have a record ready for review.”
“In the toddler/prek3 classroom, two year old children were observed and choking hazards were present in the play areas, in access to children under the age of 3.”
“One out of the eight staff records monitored did not have the in person training that is required to complete the on line certification training.”
“The annual fire inspection was observed to be expired.”
“The operation's staff did not report to Child Care Regulation, a child's injury that required medical attention.”
“The operation combined infant aged children with 18m and older children, in the music room, when combing for the 45 minutes prior to closing.”
“The outdoor monthly checklists have not been completed since May of 2025 and are not current.”
“3 out of the 5 staff records monitored, did not have their annual training current for Shaken Baby Syndrome, Sudden Infant Death Syndrome, and Early Childhood Brain Development.”
“4 out of the 10 children's records monitored, did not have up-to-date immunization records.”
“The center director did not have a current CPR and First Aid certification training.”
“The operation's staff did not make the decision to call for emergency services when a child showed an urgent need for medical care.”
“Infant children were left unsupervised when a caregiver left her classroom for a restroom break and asked a second caregiver to supervise them when that caregiver was caring for a separate group of children in a different room.”
“Children were placed in a highchair for approximately 5-10 minutes as a form of discipline.”
“One infant classroom caregiver was observed on her personal cell phone while she was supervising children.”
“10 children in the toddler/ PreK mixed classroom did not wash their hands prior to eating their food.”
“10 children were observed eating in highchairs and were observed to not be strapped to their seats that require seat straps.”
“Inspector observed one caregiver on their cell phone while caring for children. Inspector spoke to staff who stated they are not allowed to be on their cell phones when in a classroom with children. Inspector has previously provided technical assistance on staff on cell phones while caring for children during a previous inspection.”
“Inspector observed one person present and in a classroom with unsupervised access to children without an initial background check submitted. At the time of inspection the caregiver was seen in multiple classrooms.”
“Inspector observed two classrooms out of ratio. The infant room had 11 infants with one caregiver. The caregiver did not know the ages of the infants except that the youngest infant was one month old. A second classroom had one caregiver with 23 children. The children were between 2-4 yrs. old. The specified age group was 3 yrs. old.”
“Based on the information provided during the course of the investigation, two infants were observed being left asleep in car seats while in care at the operation.”
“The operation did not have annual documentation for checking the carbon monoxide, smoke detector, or fire extinguishers monthly.”
“The emergency preparedness plan was missing information such as how children with limited mobility or who are under 24 months will be evacuated, and how staff will continue to care for children until parents arrive.”
“Inspector observed two classrooms change children and not sanitize the diaper changing table after each child.”
“Inspector observed one 4 month old infant in a crib lying on a boppy pillow.”
“One staff left classroom to open the door leaving the classroom out of ratio. There was one staff with 8 infants until the second staff returned.”
“Inspector observed one 6 month old infant in a bouncer with a blanket and bottle. Staff stated the blanket was being used to prop bottle.”
“Several classrooms were observed to have chipped paint on walls which is accessible to children.”
“Two out of six classrooms observed did not have an activity plan posted.”
“One child, age five months old, was observed sitting in a bouncy seat for almost thirty minutes.”
“A caregiver in the infant room was observed feeding a child with bottle while child was sitting in a bouncer seat.”
“Two stationary play structures (one blue Thomas the Train and one red/pink fire truck) was observed with broken plastic parts. The broken plastic was observed to be sharp with jagged edges which poses a risk to children.”
“In one classroom, childrens' snack was not placed on a plate or napkin prior to serving.”
“One child was observed awake sitting in a stroller during naptime. NOTE: this was corrected at time of inspection.”
“A caregiver was observed talking to another caregiver through the door to which her back was turned toward the children resulting in one child crawling out the doorway and into the hallway.”
