Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Director's attendance records were not available for review.”
“Toddlers classroom and pre-k classroom do not have well defined centers. Toys and books are in high shelving, out of children's reach.”
“Children in the infants' room were observed to be served gold fish for snack.”
“Two year old child was observed sitting down in a bucket seat table when placed in time out. Time out lasts from 10 to 15 minutes.”
“Bottles in the toddler's room were not labeled with children's names.”
“Chicken nuggets were served whole to infants and toddlers.”
“Current activity plan for toddler class was not available for review. The activty plan posted for the class was for week of August 29th-September 2nd, 2022.”
“Diaper changing mats in two classrooms were observed to have food crumbs and yellow and grey stains. Note: Diaper changing mats were cleaned during inspection.”
“The trashcan in the pre-k classroom was observed to be uncovered. It contained left over food.”
“A group of 20 children with a specified age group of 4 years, was observed to be supervised by two caregivers. In two instances, one caregiver was observed to step out of the room leaving the group out of child/caregiver ratio. The maximum number of children a caregiver can supervise based on the specified age group are 18 children.”
“There is a diaper changing table in the pre-k classroom. The classroom does not have a sink available for hand washing.”
“There was no additonal employee in the center aside from the ones required to supervise the classrooms during naptime.”
“There were two buckets containg water outside of the children's restroom. Buckets were accessible to children. Three outlets in the pre-k classroom were uncovered. Note: Buckets were removed during inspection. Outlets were covered during inspection.”
“Six children's files were evaluated. Three files did not have the immunnization records.”
“Four employee records were evaluated. One employee completed the CPR training 100 % online. The training did not include the use of a CPR manikin and hands-on skill-based instruction, practice, and testing.”
“Two infants were observed sleeping on restrictive devices. Caregivers did not move children from devices to their cribs as soon as possible. NOTE: This was corrected as caregivers moved children to their designated cribs upon being informed by CCR Inspector.”
“A toddler child was observed to be falling asleep inside a high chair device attached to a small chair during nap time. Child was leaning towards the front of the chair. Caregiver providing supervision did not move child until being informed. NOTE: Caregiver moved child and placed him in a sleeping cot during inspection.”
“A caregiver was inside the toddler classroom assisting another caregiver on 8/10/2021. An eligibility letter for this caregiver was not received until 8/11/2021. NOTE: Eligibility letter for caregiver was received on 8/11/2021.”
