Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation did not have the personnel files available for review.”
“The operations last health insepction was 8/21/2025.”
“During a review of the parent handbook, it was observed the operation did not have anything mentioning their inclusive care services.”
“During a review of the staff records, it was observe three staff members lacking annual training hours.”
