Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A child was missing the health statement during the inspection.”
“A caregiver was missing pre-employment affidavit in the records. Note: It was corrected at the inspection time when the caregiver signed the required form.”
“There were observed some cables coming out of the ground in the playground area representing safety hazards for children.”
“The operation failed to notify Child Care Licensing of an incident in which a child required medical treatment within required timeframes.”
“A caregiver did not use appropriate judgement when grabbing a child, resulting in the child sustaining a dislocated elbow.”
“Information gathered indicates that the director has not maintained consistent presence at the operation.”
“The operation failed to ensure appropriate supervision. As a result, a child briefly exited the facility without staff supervision.”
“The operation did not provide the parents with a written incident report of the incident, as required.”
“A child was left alone, unsupervised inside the center library.”
“A 4 year old child was allowed to leave the classroom unsupervised.”
“A toddler caregiver stated there were 2 children, but there were 3 children in the group.”
“One caregiver present did not have an eligible background check,”
“A caregiver at the operation did not notify the front office immediately of a situation placing a child at risk. The operation management did not notify the parents or poison immediately of a situation placing a child at risk.”
