Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One child was missing information on the section of the health statment.”
“The backyard picket fence had two broken boards. The diaper changing mat was torn. Corrected when the mat was removed and replaced.”
“Four children had incomplete information on their enrollment form under the emergency medical attention section.”
“One parent did not signed their child's admission form.”
“At time of inspection, children were observed sitting in highchairs and were not using safety straps.”
“At time of inspection children under two were having screen time. At time of inspection, children were observed having screen time while having breakfast.”
“Two children's records reviewed lacked physician or hospital information.”
“One child lacked immunization record.”
“One child's lacked health statement.”
“A person who had direct access to children in care, did not have a background check. Note: Person is no longer present.”
“Based on the information gathered throughout the investigation, a child was not supervised appropriately when the front door was left open, and child was able to walk outside, and found by a passerby in the middle of the street.”
“Provider did not have sign in/sign out log for the day of the incident.”
“Three children's records lacked address for emergency contact.”
