Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the inspection it was found that one of the staff members who is in charge of transporting children did not have CPR training.”
“During the inspection it was found that the two drivers for the operation dose not currently have the required training hours for transporting children.”
“The operation's fire inspection is not currrent. The last inspection was conducted on 11/28/2023.”
“Two unlabeled cribs were observed in the Waddlers classroom. Note: This was corrected when the caregiver labeled the cribs.”
“The hooks for the children's belongs were unlabeled in the Butterfly and Toddler Classrooms. Note: This was brought into compliance when the caregiver's labeled the hooks.”
“Based on evidence obtained during the DFPS investigation, there is sufficient evidence to support that a caregiver did not use good judgment when the cord to a bottle warmer was left unsecured. This allowed a toddler to pull on it and sustain burns from the hot water that spilled from the device.”
“Based on information obtained, there was sufficient evidence to support two caregivers yelled at multiple children.”
“The Transition 1, Transition 2, Butterfly, and Bird classrooms all had chemicals that were within the children's reach. Note: This was corrected when the items were put away in areas out of the children's reach.”
“In the waddlers classroom, the caregiver did not know the ages of the children she was caring for and a roster with the information was not available.”
“In the Sr toddlers classroom, I observed multiple children lying on the wrong mats as the assignment chart did not match. In the Jr toddlers and transition classrooms, the mats were not labeled.”
“Five out of ten employee files evaluated had CPR certification that was taken online but was missing the hands on training instruction and testing.”
“One caregiver has a diploma that is not translated into English.”
“In the transition classroom, children were napping however, there was not adequate space between the mats.”
“One out of seven caregivers did not have CPR/first aid training on file.”
“One out of ten children did not have current immunizations on file for review.”
“Based on information obtained, there was sufficient evidence to support children were watching a program that was not age appropriate.”
