Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“It was determined a child was playing on the bar when they did not meet the required minimum age limit.”
“During an investigation it was confirmed that a child was left unattended.”
“During an investigation it was confirmed a child was left unattended and it was not reported to CCR.”
“Observed bottle with sanitizer solution without a labeled. It was corrected whent the spray bottle was labeled.”
“Observed children standing without an activity while their caregivers cleaned and put out nap mats. This was corrected when one of the caregiver's began to conduct circle time with the children in the classroom.”
“Ten children were using personal electronic devices for screen time activities that were not related to the planned activities that meet educational goals.”
“Ten children were using personal devices for screen time activities during nap/rest time.”
“The operation is serving a variety of snacks to children that do not have any nutritional value, and that exceed the allowed amount of sugar per the food program guidelines.”
“One infant was observed in a stacking crib standing up and hitting his head on the crib above his. This was corrected when the caregiver removed the infant from the crib and agreed to use a different crib for the infant.”
“Two infant cribs were observed having mattresses that were not a flat mattress that snugly fits the sides of each crib. The sheets observed being used prevents the mattress to be flat and fit snugly on the sides of the crib. This was corrected when the caregivers removed the sheets to change them during inspection.”
“The operation employed one caregiver who was attending GED classes and is under the age of 18 who was not enrolled nor completed a childcare career program.”
“A caregiver who was under the age of 18 was left alone with sole responsibility of caring for children.”
“The director assigned a caregiver assignments that did not match their skills, abilities, and training.”
“The operation employed a caregiver who has graduated from high school and is under the age of 18 who was not enrolled nor completed a childcare career program.”
“One cabinet inside of the children's classroom was unlocked allowing toxic chemicals, specifically, Febreeze, to be left accessible to children. This was corrected by the director moving the chemical to make it inaccessible to children.”
“One caregiver was present in the classroom with children without an eligible background check. The caregiver left during the inspection.”
“The operation had five trash cans inside of the classrooms that contained garbage in them without a lid or front cover allowing it to be accessible to children. This was corrected at inspection when the trash cans were removed from the classrooms.”
“One caregiver did not know the number children they were responsible for in the classroom.”
“There was one employee file out of the ten employees that were viewed that did not have a notarized affidavit for employment on file. Note: This was corrected at inspection when the employee got her affidavit notarized while at the operation, and the affidavit was placed in her file.”
“There were four child files out of the ten files viewed that did not have updated immunizations on file. However, three of those children's immunizations were corrected at inspection. There is one child out of those four that is missing an updated immunization record.”
“There was an expired Epi Pen on file for a child with a known allergy. The Epi Pen expiration date shows 1/2023.”
“1 out of 2 buses safety alarm did not work properly.”
“During an investigation there was sufficient evidence to support a child was neglected.”
“The 2-caregivers present were both observed cleaning the classroom instead of supervising the children. The classroom door was opened, resulting in a child leaving the operation near a busy road for approximately 30 minutes, without their knowledge.”
“3 cribs, used by napping infants, were not labeled with the infants name.”
“Out of 5 infant feeding instructions reviewed 4 had not been updated in the past 30 days. 2 of the forms had not been updated since January and one infant did not have a form.”
“The toddler classroom did not wash hands after coming in from outside play.”
“A bottle of bleach water was located on the back of the children's sink. This was moved by the caregiver when it was brought to their attention.”
“During an investigation it was found that the 2:00 pm diaper change was not done resulting in children being in the same diaper for almost 4 hours. There was a substitute in the classroom that day that is no longer employed.”
“During an investigation it was found that a caregiver did not use positive method's of discipline when redirecting a child's behavior.”
