Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation did not report to CCR the number of employees who left in 2025 before January 15, 2026.”
“The operation has not designated a qualified director.”
“Two infants was observed sleeping in a restrictive device. Note: This was corrected when the caregiver removed the infants from the restrictive device and placed them in a crib.”
“The operation failed to notify CCR that the director at the operation changed. The operation does not have a qualified director designated.”
“Two out of the six children's records are missing the physicians' signature of the health care statement.”
“Three infants were observed in chairs with straps that were not fastened.”
“The parent handbook provided at the time of inspection did not include policies regarding activities.”
“A child was observed sleeping in a crib with a bib around the their neck. This was corrected at inspection when the caregiver was asked to remove the bib and the bib was removed.”
“Licensing was not notified of a positive test result for COVID, a communicable disease, at the operation. This was corrected at inspection when a verbal report was provided to the licensing representative. The director will follow up with additional information reqeusted via email.”
