How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Paperwork — records, postings, forms Standard — supervision, ratio, training Health & safety — direct risk to a child

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

746.3701 — Safety - Areas Free From Hazards

“After being aware of a child who was able to open the playground gate with ease on more than one occasion, immediate action was not taken to better secure the gate. Video of an incident was observed where the child opened the gate and ran before being retrieved by the caregiver on 3/25/26. At the time of the investigation inspection on 4/2/26, it was observed that a correction to better secure the gate had not yet occurred, which showed continued risk.”

746.1201(1) — Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

“A caregiver did not use good judgment or competency in how they communicated with a parent after their child received an accidental injury that was above minor. The parent was provided with insufficient detail of the injury and did not indicate how serious the injury was. As a result, the parent was not able to accurately access their child's injury and that it needed more immediate attention than waiting until pick up time. The child had several abrasions across the face, a bruise and later began to display signs of a possible concussion prior to being picked up by the parent. The caregiver also did not ensure that the director, who was still present at the time, was properly notified. The director was not made aware of the incident until after the child was picked up by the parent.”
