Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation does not have a designated program or site director as of October 5, 2026.”
“One staff is missing documentation of their pre-service.”
“There is no fire drill documented for September. This was corrected at inspection when they conducted a fire drill and documented it.”
“One staff is missing documentation of their orientation.”
“There was no current monthly checklist available.”
“1 of 3 conditions put in place for the waiver is not compliant. "The program director will be at the operation a minimum of once a week for no less than 2 hours to ensure the site director has control of the site and an understanding of compliance with the minimum standards. There must be a log for the program director to sign in to show she is present; this must be available for CCR to review."”
“4 of 5 personnel files did not have proof of orientation available for review at the time of the inspection.”
“1 of 5 employees, who is 17 years old, does not have proof of being enrolled in a program allowing her to be eligible to work in a licensed facility.”
“1 of 5 emplyee records did not have a notarized affidavit available for review at the time of the inspection.”
“5.25hrs was observed for 1 caregiver. This is 9.75hrs less than the required amount.”
“3 of 5 personnel records were not available for review at the time of the inspection.”
“One allergy form did not have a physicians signature.”
