Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“It was observed that the bottle of a 5 month old infant was propped with a blanket. The bottle was removed during the inspection.”
“Two of four children's files reviewed were lacking updated immunization records.”
“Two of four children's files reviewed were lacking physician signed health statements.”
“One of four personnel files reviewed was observed to be missing a valid photo ID, Affidavits 2985 & 2912, and a high school diploma.”

“Infant feeding instructions for two infants were last updated in March.”
“One infant, 4 months old was observed in a classroom with a 2 year old child creating an age gap over 18 months. The two year old was moved up to the older classroom during the inspection bringing the deficiency into compliance.”
“A small slide was observed on the playground without the required loose fill. The operation will be receiving loose fill for the playground.”
