Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation did not have a record available to review of the most recent fire inspection.”
“Ten of the children files did not have all of the required admission information.”
“The operational policies did not include the promotion of physical activity.”
“During the review of staff records it was found that 3 out of 6 caregivers were missing their record of education qualification.”
“The director did not complete the required 30 annual training hours.”
“During the review of children's records 4 out of the 6 records evaluated were found to be missing the emergency contact address.”
“One caregivers' education document was not translated into English.”
“Three volunteers did not have the required background check submitted. One volunteer was serving in the role of a caregiver counted in ratio. The caregiver role requires fingerprints to be submitted.”
“Three caregivers did not receive Pre-Service Training in the area of Fostering Children's Self-esteem.”
“I observed 2 out of 3 children's food allergy emergency plans that did not include the health care professional signature.”
“The operation emergency preparedness plan does not provide a name and adress to an alternate shelter.”
“Five employees did not have in-person CPR Training. The training on record was obtained through an online source not allowing for use of a manikin.”
“One volunteer and a caregiver didn't have a eligible background check. The background is still in pending status.”
“Three volunteers at the operation didn't have a background ran on them. Two caregivers in the three-year-old class and one caregiver in the toddler classroom.”
“The operation did not have a updated fire inspection.”
“The operation didn't have fire drills document from last school year. Corrected: The director filled in the documentation by using a calendar she would use for her schedule the days she would do the drills.”
