Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“There was a child's rescue medication that was expired on 10/2025.”
“In the older infant classroom, after a diaper change the child's hands were not washed. NOTE: This was corrected when another caregiver came in to assist.”
“At the time of the inspection the liability insurance available expired 1.29.25.”
“The annual health inspection was last conducted on 12.19.24.”
“One of the ten children was missing parent's name, telephone number. One of the ten children was missing release information. Two of the ten children were missing field trip permission. Two of the ten children were missing special care statement.”
“Four of the ten children were missing healthcare statements. Three of the ten children files were missing immunization records.”
“During the course of the investigation, one out of the four caregivers did not have an updated CPR/First Aid certification.”
“A staff member failed to use good judgment when as a consequence of behavior, she forced a child to pick up toilet paper with her bare hands from the restroom.”
“During the course of the investigation, it was verified a staff member will yell at children when redirecting them.”
“Operation video footage demonstrated an operation staff member grab a child twice while redirecting.”
“At time of inspection, safety child alarm did not work in two of the three transportation buses for operation.”
“At time of inspection two diaper changing stations were observed with debris.”
“Operation did not enter information by due date 1/15/24.”
“A toddler restroom floor wall base was observed to be detached from the wall. At time of inspection diaper changing table was observed to have debris. Corrected during inspection.”
“At time of inspection one infant caregiver did not have updated annual training in Recognizing and preventing shaken baby syndrome; Preventing sudden infant death syndrome; and Understanding early childhood brain development.”
“During inspection, after school class had 37 children with 2 caregivers, max group size is 35. This was corrected at inspection.”
“At time of inspection 2 infants were observed asleep in a restrictive device. This was corrected at inspection.”
“During inspection it was observed there were roots protruding from the ground in the fall zone of the play equipment, creating a hazard.”
