Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“2 expired items were found in the first aid kit. This was corrected at inspection when the items were removed. Non-expired items were avialable in their place.”
“According to the information collected throughout the investigation, it was concluded that a caregiver forcefully grabbed a child by the face and arm.”
“It was concluded that the operation failed to submit a report to CCR in a timely manner when a caregiver employed inappropriate disciplinary measures against a child.”
“According to the information collected throughout the investigation, it was concluded that a caregiver yelled at a child.”
“During the course of the investigation, it was determined that a caregiver fell asleep while supervising children.”
“It was determined that the operation did not make a report to CCR when a caregiver fell asleep while caring for children.”
“Based on information gathered during a DFPS investigation, a child was left unsupervised for an undetermined amount of time.”
“Based on information gathered during a DFPS investigation, it is determined that a child was able to access hazardous materials and subsequently sprayed a diluted cleaning solution into their mouth.”
“Based on information gathered during a DFPS investigation, the operation was aware of an allegation that placed a child at risk and did not report it to CCR.”
“The operation has three play structures and three moved slides that require at least 6 inches of loose fill. All use zones were measured between 0-1 inches of loose fill.”
“The operation does not have a CO2 install in the building.”
“An 11 month old was observed in a play yard asleep with a cup and nap roll (pillow and blanket). CI: The caregiver removed the cup and nap roll from the play yard during inspection.”
“An walking infant was observed in a stackable crib and their hit was hitting the top of the crib when standing. CI: The director resigned the child's crib during inspection.”
“The furniture in the infant room is arranged in such a way that it does not allow for caregivers to see infants at a glance from all areas of the room.”
