Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Three children were sitting in bucket seats without any straps.”
“The gate to the playground did not close properly and has an open area. The fence in the school age playground is coming down. The window in the bus is broken and covered with plastic. There are tiles in the school age room that have been covered due to the ceiling leaking.”
“One changing pad has tears in it and the other had a cloth cover which was removed at the inspection.”
“The operation did not report a positive COVID-19 diagnosis within the required 48 hours.”
“This standard was evaluated due to an investigation and found to be in violation. Improper maintenance to gate allowed a child access to get out of gate unsupervised.”
“This standard was evaluated as part of an investigation and was found to be in violation. The operation did not provide an incident report to the parent within 48 hours.”
“This standard was evaluated as part of an investigation and found to be in violation. The facility staff failed to supervise a child appropriately. A 14 month old child was found alone in a street near the operation. A passerby found the child and the staff did not know the child had left the group.”
“This standard was evaluated during an investigation and was found to be in violation. A child was neglected while in care at the operation.”
