Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on a DFPS Investigation the operation did not notify a parent of an injury during pick up.”
“Two out of ten student files did not have updated immunizations.”
“Emergency contact infromation was missing for one out of ten student files.”
“Five out of ten student files are missing a health statement.”
“Four out of ten employees were missing an updated affidavit.”
“One out of ten student files were missing medical authorization.”
“This was found to be deficient when two four-year-olds were left unsupervised.”
“This standard was evaluated as part of an investigation and a deficiency was found. A caregiver inappropriately grabbed a three-year-old child under their arm.”
“This standard was evaluated in regard to an investigation and a violation was found. There is sufficient evidence to support the allegation that a caregiver pulled a child off the ground by the arm.”
“This standard was evaluated in regard to an investigation and a violation was found. There is sufficient evidence to support that a caregiver was yelling at children.”
“The operation failed to report a positive case of covid to the department. This standard has been corrected. I discussed with the director reportable situations and timeframes.”
