“At inspection, four caregiver files did not have the required 2912, pre-employment affidavit form and one caregiver file was missing the 2985 notarized affidavit form.”

“The operation has a climbing structure with a slide that does not have proper use zones. The slide has a climbing wall in it's use zone. The use zone in front of the slide does not contain a full six foot zone of surfacing material.”

“6 staff were missing the following required content trainings: SIDS, Admin Medication (except 1), Food Allergy, Hazardous Material (except 1).”
