A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“During the inspection, it was discovered the assigned site director was no longer working at the operation, and the program did not notify CCR within the required time frame. There is also a new site director and the operation did not notify CCR.”
“The operation did not notify CCR of the change in controlling persons within the required time frame.”
