Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“All the required information for abuse and neglect was not including in the parent handbook such as warning signs, trainings, prevention methods, and community engagement/resources.”
“A new hire non-exempt caregiver who requires pre-service training has not obtained the first 8 hours of pre-service training before being counted in child/caregiver ratio.”
“Parent rights were not fully included in the updated parent handbook.”
“One caregiver's education documentation was in a foreign language and not translated.”
“Two new hires for the toddler room did not have first aid training prior to having unsupervised access to children while in care.”
“Based on DFPS documentation obtained, proper supervision was not maintained when 3 children were able get out the toddler playground gate and into the parking lot unsupervised.”
“It was self-reported and found that a 1 year old was inappropriately disciplined when a caregiver hit the child's hand out of punishment.”
“Children under the age of 4 were served whole wieners, whole grapes and whole cherries which is a choking hazard.”
“One caregiver did not know the ages of the children they were caring for and did not have this information showing each child's age.”
“Children were sitting in seats and were not strapped in. The seats were missing the safety straps. This was corrected at inspection.”
“The director reported that a caregiver was seen using inappropriate discipline with a child in care. Based on the evidence, the allegation was supported. The caregiver responsible is no longer employed at the daycare. An emergency meeting was held with all remaining staff and standards were reviewed, and staff were given additional training on appropriate discipline to help ensure there are no future incidents.”
“Upon follow up, the fire inspection was still not completed. The inspection was requested and paid for but the inspector has not yet arrived. An extension is granted.”
“Upon follow up, the fire inspection was still not completed. The center is struggling to get the name changed on the account and needs more time to arrange for an inspection. An extension of 2 weeks is granted.”
“It was alleged that a caregiver used inappropriate language with a child. Based on the information obtain during the investigation, there was support for this allegation.”
“The fire inspection had expired in March 2022.”
