Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the evidence obtained during the DFPS investigation there is sufficient evidence to confirm that a caregiver did not utilize good judgement as they grabbed/pulled a child by the arm. A caregiver pushed a child once causing the child to bump into a nearby shelf, twice causing him to fall on his buttocks on the floor. The caregiver also slung the child by the arm, and the child fell forward onto what appeared to be a cushioned mat.”
“During the investigation it was learned that the operation failed to ensure the parent was timely informed of all known information regarding a situation that placed a child at risk. Although management later identified possible causes of the injury through video review, including a block striking incident and a slip-and-fall, this information was not promptly communicated to the parent.”
“During the investigation it was discovered that the operation failed to accurately document all known information regarding a situation that placed a child at risk. After management reviewed video footage and identified additional events that may have contributed to the child?s injury, the incident report did not initially reflect those findings and required revision. Therefore, the Licensing Incident/Illness Report Form was incomplete and did not accurately document the circumstances known to the operation.”
“The assistant directors did not have access to their personnel files in the absence of the director for CCR Inspector to review.”
“A child's admission form did not include release information. The admission form is blank under the release authorization.”
“The operation was cited due to lack of supervision during a transition period which they admitted to leaving a child behind in a classroom unattended.”
“A child in care was left unsupervised in the classroom”
“During the investigation staff did not know the children they were responsible for.”
“According to DFPS investigation findings video footage was reviewed and showed caregiver grabbing a child by the arm.”
“Based on the information obtained during the investigation, the operation is deficient regarding a child left the classroom and accessed the outside playground.”
“During the walk through two rooms had chemical bottles accessible to children and two rooms stored chemical with food items. Note: This was corrected at inspection when the chemicals were moved and stored properly. Pictures were taken.”
“Based on the evidence gathered during the investigation, it was found that a child was left unattended.”
“Four out of ten staff records reviewed completed CPR training online.”
“A caregiver used their hands to "smush" an infant's face as a form of discipline.”
