A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“During the investigation, it was confirmed the children have had access to space on the second floor of the building despite the operation never receiving written approval from the local fire authority to care for children above ground level.”
