A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“During a follow up inspection, it was obseved a movie being played were two children of 18 months old were present.”
“During walkthrough it was obseved two children less than two years old watching cartoons on the television.”
