Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation failed to provide CCR with a signed copy of the incident report as required.”
“The operation failed to communicate the severity of the child? s injury to the parent and convey the need for immediate medical attention.”
“A report of a child in care being injured at the operation was made to Licensing 8 days after the incident occurred.”
“Based on information obtained throughout the investigation inappropriate discipline was practiced when a caregiver pulled a child across the floor causing redness to appear on the child's rib cage.”
“During the walk through in one classroom the caregiver did not know the number of children they were responsible for. Caregiver stated 11 and there was 12 children.”
“Based on the DFPS investigation and after reviewing the camera footage, it was found that the caregiver was not supervising the children. Children were observed climbing on tables, a child pulled a chair that was holding the restroom door open, and the child inside was left in the restroom and the caregiver was not interacting with the children.”
“Based on the DFPS investigation and after reviewing the camera footage, it was found that the caregiver was cleaning and sweeping the classroom and it took her attention away from supervising/interacting with the children.”
“Based on the DFPS investigation, it was found that the operation staff did not communicate with the caregiver regarding child information and information about which children were potty trained.”
“In the infant classroom the caregiver did not wash the infants hands after changing the child's diaper. Note; When pointed out the caregiver washed the child's hands.”
“3 staff counted in ratio during the inspection did not have active background checks on file. They had been inactivated on 9/15/22 and 12/12/22 respectively. Note: The background checks were submitted during the inspection.”
