Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A four-month-old, non-walking infant was observed sleeping on the floor in the playroom.”
“One caregiver was observed to step out of her class to open the building door, leaving class unsupervised. Second caregiver was observed to be walking back to her class.”
“Four employee files were evaluated. CPR training has been completed 100 % online. Training did not included hands on practice on manikins.”
“Medication was within children's reach in pre-k classroom.”
“Two slides did not have the proper depth of surfacing and use zone.”
“Two infant rooms written schedule did not have two daily outdoor opportunities. This was corrected at the time of inspection.”
“A 9 month old and a 10 month old were observed to be in care with a 3 year old. Note: This was corrected at the time of inspection. Infants were moved to an infant room.”
“In two classroom's televisions were not anchored down and on the counter in the classrooms.”
“Screen activty was viewed in the infant room 0-12 months.”
“Upon inspection there was only one toilet working out of three.”
“At the time of inspection, the infant room had a four month old and a two year old counted in ratio, with a 21 month age difference.”
“The operation lacked an additional caregiver in the building during nap time.”
“At the time of inspection, a non-walking 11 month old was placed on a mat for naptime.”
“At the time of inspection, a 10 month old was observed sleeping in their crib with a blanket, and a 11 month old sleeping in a swing with a blanket covering their face. *Both blankets were removed from the infants.”
