Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Three infants were combined with the toddler class outside. The youngest child in the group was 16 months and the oldest child was 35 months. This was corrected during the inspection when the infants went back inside to the infant room.”
“A substitute file was reviewed, the personnel's file was missing: (1)Documentation showing the dates of the first (2)Documentation showing how the employee meets the minimum age and education qualifications (4)A notarized Licensing Affidavit for Applicants for Employment form as specified in Human Resources Code, ?42.059 (5)A record of training hours, including documentation required by ?746.1329 of this chapter (6)A statement signed and dated by the employee showing he has received a copyof the child-care center s: (A)Operational policies[Medium]; and (B)Personnel policies”
“Children 18months to 2 years old were observed outside on a walk in a non-closed area. The Director brought the children inside and reviewed the ratio and group size standards for walking field trip standards with the caregivers. The Director stated the children will only go outside the fenced area, if the operation has enough caregivers to accommodate the max group sizes for proper supervision.”
“There were 2 infants observed in a chair that is equipped with safety straps and the safety straps were observed to not have been fastened. The caregiver and director removed the children from the chairs.”
“While doing a walkthrough there were two children under the age of twelve months sleep in their cribs with bibs on. The bibs were immediately removed.”
“While reviewing the operational policy there was no promotion of indoor and outdoor play available in the policy.”
“While doing a walk through there was a bottle of disinfectant cleaner on the sink in a restroom used by children. This was corrected during the inspection when the bottle of cleaner was put in a high cabinet away from children.”
“The grounds used for outdoors play requires maintenance as there are tripping hazards. There were several gaps beneath the turf material used on the ground, which could lead to injuries if children trip and fall. There are concerns that during play the children have access to bird droppings and leave that could be put in their mouths, which could be a risk to their health.”
“The fence on the playground does not meet the requirements for four feet all the way around the playground space.”
