Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“2 out of 10 children's files reviewed did not have a health statement.”
“The director did not have current transportation training.”
“An infant's bottle was not labeled. This was corrected at the inspection when the caregiver labeled the infant's bottle.”
“3 out of 5 infants did not have updated feeding forms.”
“This standard was evaluated in regard to an investigation and a violation was found. The director grabbed one child by the face and another child by the arm and pulled them down in an inappropriate manner.”
“This standard was evaluated in regard to an investigation and a violation was found. There is sufficient evidence to support that 2 different caregivers disciplined 2 different children by inappropriately grabbing the children.”
“This standard was evaluated in regard to an investigation and a violation was found. The operation was allowing caregivers unsupervised access to children without first aid training.”
“Numerous children's records were found to be missing the Health Statements in their admission forms.”
“One child did not have current immunizations at the time of inspection.”
“Based on information obtained during an investigation, it was determined that a caregiver did not use a positive method of redirection to address undesired behavior. A caregiver re-directed a child by picking the child up by the arms in order to move them from one area of the classroom to sit them down in a chair.”
“The operation does not have a written activity plan for infants in care.”
“Infants are not given an opportunity to go outdoors.”
“A child was not given medication as amended by the child?s health-care professional.”
“This standard was evaluated as part of an investigation and found to be deficient. A child was administered medicine without obtaining medical authorization from the child's parent.”
“The Licensing keeping children safe poster,The Licensing Parent Notification Poster and required emergency phone numbers were not posted at the time of inspection. This deficiency was corrected at inspection when the permit holder printed and posted required postings”
“Children were not offered a fruit for breakfast. One infant was given water instead of milk at breakfast.”
“The use zone for the exit of one slide on the playground used by children is only four feet.”
