Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the walkthrough of the operation one caregiver was observed on her personal phone.”
“Based on the evidence and information obtained during this investigation, it has been concluded that the operation failed to provide age-appropriate resting equipment to several school age children.”
“During nap time two children were seen laying down with out a mat. NOTE: This was corrected at inspection when caregiver provided the children mats.”
“There is a slide to be used by children that has been installed over asphalt and does not have the proper surfacing. (NOTE: Picture on file)”
“One of the children's files indicates that there is a food allergy; however there is no allergy plan on file.”
“Out of the child's records reviewed, 2 had incomplete information for the person to contact in case of an emergency.”
“Out of the staff files reviewed, staff are missing the remaining 16 hours of pre-service training.”
“Out of the children's files reviewed, 4 had incomplete information for the Dr/Hospital.”
“There is no current health inspection on file. The last one was done August 2021.”
“Upon arrival there was a video playing in the main area where all children were gathered. There were 3 children under 2 yrs of age in the same group. (NOTE: Corrected during inspection by turning off the video)”
“There were infant feeding schedules that had not been updated in the last 30 days.”
“A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.”
“One of the children's files shows that there is no health statement on file. The child was enrolled on 12/7/21.”
