A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“During the inspection there was not any reviewable documentation of when the operation last conducted a fire drill. This was corrected at inspection when a fire drill was conducted while state licensing was present in the home to meet compliance.”
“During the inspection the primary caregiver did not have all of their required annual training hours completed.”
