Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During a DFPS investigation, children did not wash their hands before eating.”
“During a DFPS investigation, children did not wash their hands after entering the operation from outdoor activities.”
“During a DFPS investigation, one caregiver, who was counted in ratio, was observed to be on their cell phone on two occasions. On the first occasion, this caregiver was on their phone for 15 minutes and then this same caregiver was on their phone later in the day for six minutes. The use of the caregiver's cell phone use was not directly involving the teaching, care, and supervision of children.”
“Two slide structures are out of compliance. The slide structures did not have the correct surfacing, the amount of surfacing and one slide structure had screws that were too large, becoming a protrusion.”
“In the playground there was debris that was not picked up and a wading pool that was broken in the sand. The playground was cleaned and item was removed at inspection.”
“During a DFPS investigation, a caregiver was found Reason to Believe for Physical Abuse of a child in care. This caregiver was forcing a child to nap; as the caregiver was physically handling the child, the caregiver caused the child's elbow to break.”
“During a DFPS investigation it was determined that a caregiver was forcing a child to nap.”
“An 11-month-old child got injured at the operation and none of the staff that was in the classroom or staff that entered to provide breaks to the staff were not aware of the injury and did not notice how the child got injured.”
“An 11-month-old child who is not mobile was sleeping in a mat instead of a crib during naptime.”
“An 11-month-old child who is not mobile was placed in a toddler chair with no straps during breakfast time for approximately ten minutes.”
“One child did not have the information of name, address, and telephone number of the child s physician or an emergency-care facility on file.”
“One child did not have a copy of the immunizations on file for review.”
“A caregiver had five infants under age of 12 months. The specified age was 6 months. The caregiver was over ratio by one child. This was corrected when a caregiver arrived to the center and helped the infant teacher with in ten minutes. Classroom was in ratio.”
