Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“This standard was evaluated as part of an investigation and determined to be deficient. It was determined that a caregiver inappropriately grabbed and pulled a child by the arm.”
“This standard was evaluated as part of an investigation and found to be deficient. A caregiver smoked/vaped inside of a bathroom located inside the facility. The caregiver was terminated immediately after the administration was made aware of the incident.”
“Based on information provided by DFPS, it was determined that during the walkthrough of the operation, an infant was sleeping in a bouncer seat.”
“Based on review of video and interviews with staff, it was determined that a caregiver did not demonstrate good judgment and competency during naptime when the caregiver roughly patted two young children on their backs.”
“This standard was evaluated as part of the investigation and was found to be deficient. On the weekly menu provided by the operation, the menu did not state that milk or a milk substitute is being served during lunch. In addition, on three of the five days during the week, items from only one food group were being served at snack times.”
“I observed that there was no activity plan posted in the school age clasroom. The director corrected this at the inspection.”
“I observed that the playground equipment did not have the appropriate use zone where placed in the outdoor space. It needed to be moved an additional foot from the fence to ensure a 6 foot use zone. The director corrected this during the inspection.”
