Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The gun and ammunition were in the same locked apparatus. This was corrected at the inspection when the director removed the ammunition and placed it in another locked apparatus.”
“The indoor active activity involved the use of the TV with a music video for active movement. There were 8 children under 2 years old in care. This was corrected at the inspection when the director placed a cover over the TV.”
“A 10 month old was placed in a play yard which had 2 additional mattresses in it. This was corrected at the inspection when the director removed the infant and the mattresses.”
“On the admission form, a parent indicated a child had a diagnosed allergy. The emergency care plan in the child's file was not signed by a physician.”
“One child's record lacked the updated immunization record for age.”
“The last fire drill conducted was on 09/22/2022.”
“The child/caregiver ratio was not met. The director had 7 children in care ages, 1-10 months, 1- 11 months, 1-13 months, 1-16 months, 1-20 months, and 2- 2years old. With 4 infants, the director may care for 2 additional children. The director was over child/caregiver ratio by 1 child.”
“The assistant caregiver's file lacked the educational requirements.”
“There were no written feeding instructions for the infants in enrolled and in care.”
“The operation had a 1A 10BC fire extinguisher not the required 3A 40BC.”
“There was no carbon monoxide detector upstairs.”
“The was a fence board missing.”
