Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One staff file reviewed did not have all the required health and safety trainings.”
“During inspection, classroom was observed out of ratio. The room had 1 caregiver to 7 children. The room was out of ratio by one child. Note: This was corrected during inspection when a child left the operation and center was in ratio.”
“At time of inspection two out six children files had parent as emergency contacts.”
“Operation did not enter information by due date 1/15/24.”
“1 out of the 3 personnel records reviewed did not have documentation of how the caregiver met the minimum education requirements.”
“1 out of the 4 children's records reviewed did not have a Healthcare Provider Statement on file.”
“1 out of the 4 children's records reviewed did not have the emergency medical care authorization filled out by the parent.”
“One of the infants enrolled had a feeding schedule, but it was not signed and dated by the parent/guardian. Note: This was corrected during the inspection when a signed and dated copy was provided to the operation by the parent.”
“None of the infant cribs were labeled either with the child's first name and last initial or numbered and accompanied with a number chart. Note: This was corrected during the inspection when all of the cribs were labeled with the child's first name and last initial.”
“The bottles for 2 out of the 2 infants present were not labeled with the infants first name and last initial. Note: This was corrected during the inspection when all of the bottles were labeled.”
