Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“Four children records did not have the information for a physician or an emergency care clinic in their file.”

“The provider has a household member who was present at the operation. The household member has an ineligible background check at this time and cannot be at the operation.”
