Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The nap cots were not labeled. This was corrected at the inspection when the nap cots were labeled with a number and each child was assigned a specific number.”
“3 of the 6 playpens observed did not have proper fitting mattresses. This was corrected at the inspection when the director replaced the playpens with cribs.”
“During the inspection, a stool used by children was observed with dirt and dust. This was corrected during the inspection when the stool was wiped down and sanitized.”
“A caregiver with a conditional background check was observed alone with a group of children.”
“The diaper changing mat in one of the infant classrooms was observed with rips and tares. This was corrected at the inspection when the caregiver replaced the diaper changing mat.”
“During the investigation it was found that a caregiver did not use good judgement by following a parent to their car and using profanity during the hours of operation while children were in care.”
“The playground area had one exit. The area is required to have two exits.”
“One of the infant rooms is not well ventilated.”
“The children's product safety form was not updated. This was corrected at the inspection when the director filled out an updated form and posted it.”
“The operation started providing transportation from school prior to notifying CCR.”
“An infant was observed sleeping in a play yard with a bib around their neck.”
“During the investigation inspection there was no caregiver in the infant room.”
“An infant was observed receiving a diaper change without washing their hands after. This was corrected at the inspection when the caregiver washed the infants hands.”
“An infant was observed sleeping in a restrictive device. This was corrected at the inspection when the infant was placed in the crib.”
“The crib did not contain a firm flat mattress. This was corrected at the inspection when the director put a mattress and firm fitted sheet into the crib.”
“A caregiver with a pending background check was observed in a room with children.”
“During the inspection there was a person present with direct access to the children before their FBI fingerprint results were in. The person was being counted into ratio.”
“An infant was observed sleeping in a bouncer.”
“During the inspection, there was no caregiver present in the infant room.”
“During the monitoring inspection, 2 infants were observed in a restrictive device for more than 15 minutes.”
“During the inspection 1 of personnel records reviewed did not contain the 24 hours of annual training required.”
“During the inspection, a caregiver was observed texting on their cell phone during the children's outdoor play time.”
“During the inspection a sharp metal board was observed sticking out from the side of the building on the playground area. This was corrected at the inspection when the caregiver placed duct tape around the sharp areas.”
“A review of the Employees Who Left Employment in 2022 data from January 26, 2023, revealed that the center had not reported the number of employees who left employment in 2022, as required.”
