Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The caregiver from Pre-K 1 classroom did not know the ages of the children they're responsible for when being asked by CCR representative. Ths was corrected.”
“The first aid kit in the bus was missing the thermometer and the tweezers. Note: The operation placed the first aid kit with thermometer and tweezers in the bus.”
“Training certificates showing caregivers have completed the full 24 clock hours of annual training was missing for 6 out of 10 staff files evaluated.”
“The director of the operation provided training to staff within two year period of being assessed an administrative penalty.”
“8 out of 10 staff files evaluated were missing the Pre-Employment Affidavit Form 2912.”
“3 out of 10 children's files did not contain the address to the emergency contact person.”
“The operation did not have the emergency medical treatment and transport information for children being transported.”
