Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two children climbed onto a shelf when one child pushed the other off, resulting in a serious injury. It was determined that the incident occurred due to insufficient supervision.”
“A caregiver was engaged in janitorial duties to the extent that they did not notice a child with a choking hazard in their hands and mouth totaling approximately 3 minutes. The parent found their child with the choking hazard during pick and notified the caregiver.”
“3 employees did not have the required pre-employment affidavit 2912 on file.”
“An outdoor plastic tree house used for active play had other play equipment within the use zone area. This was corrected during the inspection by the permit holder removing the items from the use zone.”
“An outdoor plastic tree house used for active play did not have the required surfacing, such as loose-fill or unitary surfacing.”
