Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Children files are missing information regarding Authorization For Emergency Medical Attention section.”
“Child's file is missing a health care statement.”
“The provider and one household member did not have documentation of orientation on file.”
“The provider was missing 11 hours of annual training for the year 2023-2024.”
“One child present at the operation did not have admissions paperwork available for review.”
“The operational policies on file were missing information on parents vising the home without prior approval, reviewing minimum standards or obtaining them on line, child care regulation website, promotion of indoor and outdoor activities, and the emergency preparedness plan was also missing information such as how a child with limited mobility or who are under 24 months will be evacuated and how the provider will continue to care for children until parents arrive.”
“The fire extinguisher was found in the home on a counter in the kitchen unmounted.”
“Three out of four children's records did not have a date of admission.”
