Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The parent handbook was missing procedures for inclusive services for special needs children, information on parent's rights, and a discipline and guidance policy.”
“Out of four children's files reviewed, two children were missing permission to ride the school bus to the operation, a statement from the parent that they do not have any allergies, and a statement from the parents that their immunizations were on file with the school. One child was missing a statement from the parent that they received a copy of the operational policies.”
“The director and three caregivers were missing training in communicable diseases for the 2023-2024 training year.”
“Out of four staff files reviewed, two were missing proof of education and one was missing a notarized affidavit.”
“There was sufficient evidence found during the investigation to show that children were not being adequately supervised when lining up to use the restrooms at the operation.”
