Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A child was left unsupervised by a caregiver at the operation.”
“In the DFPS Investigation, a caregiver was found to have physically abused a child in care at the operation.”
“In the documentation completed as part of a DFPS investigation, it was found that a child in care had been bruised when a caregiver pinched him.”
“In the documentation completed as part of a DFPS investigation, it was found that a child in care had been yelled at by a caregiver.”
“This standard was evaluated as part of an investigation and determined to be deficient. It was determined that a bus driver did not have a current bus list when transporting children.”
“This standard was evaluated as part of an investigation and determined to be deficient. Through interviews with operation staff, it was determined that the operation failed to notify a parent when their child was involved in a non-routine situation.”
“The operation did not have a passed health inspection. They have made the repairs required by the county health department and have scheduled the health inspector to come back to the operation.”
“The surfacing material for the slides and climbing equipment was 4-5 inches in depth. The operation has ordered additional surfacing material which has not yet been shipped to the operation.”
