Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One of the caregivers did not have at least 15 hours of training for the last complete training year of August of 2024 through August of 2025.”
“Based on the evidence obtained during a DFPS investigation, a written incident report was not completed as required when a serious incident occurred that resulted in 2 children leaving the facilities property unnoticed by the operation's staff.”
“Based on the evidence obtained during a DFPS investigation, it was found that children in care were not supervised appropriately. As a result of this, 2 children were able to wander away from the facility property.”
“It was found during the investigation the operation did not report an incident that occurred that required a child to receive medical treatment.”
“It was found during the investigation that a caregiver who was under the age of 18, attending high school and has no proof of enrollment or completion of a child-care related course was left alone with sole responsibility of caring for children.”
