Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Through investigation, it was determined that a child was left alone in a classroom.”
“A person with a pending background check status was observed to be in a classroom with children during naptime.”
“Through investigation, it was determined that an incident was not reported to CCR as required.”
“Through investigation it was determined that a child was left unsupervised in class.”
“Two child files were observed to be in need of updated immunization recrods.”
“Four infants in care were lacking current signed feeding instructions.”
“Two staff files were observed to be missing a Preemployment Affidavit (Form 2985).”
“A food allergy emergency plan was not present in the file of a child who was identified as having a food allergy.”
“A current health inspection report was not available at the time of inspection,”
“A caregiver allowed a child to remain in the bathroom alone, where the child could not be seen. placing the child at risk of harm.”
“One caregiver file was observed to be without documentation of orientation training.”
“A caregiver was observed on video recording using inappropriate discipline, including grabbing, poking, and yelling at children.”
“The operation did not have a log verifying that the carbon monoxide detector is being checked monthly. The Director purchased a battery operated detector that can be monitored monthly, and ran a test and logged the test on a form during the inspection.”
“One infant did not have a current feeding instruction document in the classroom signed by the parent for the month of September 2024.”
“The operation did not have a record verifying monthly checks of the equipment and surfacing material for normal wear and tear, broken or missing parts, debris or foreign objects, drainage problems, or other hazards. The operation Designee completed a check of the play area in detail and documented the findings during the inspection.”
“Based on the information obtained, it was determined that a child was being left in restrictive devices for longer than the 15 minutes allowed.”
“The operation failed to report the number of employees left in 2023 by the deadline of 1/15/24.”
