Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver provide the wrong amount of children they were responsible for during the walk through twice.”
“The operation has multiple missing and water damaged ceiling tiles that need to be replaced.”
“A caregiver was observed flicking a child.”
“A caregiver was observed flicking a toddler on the head and another caregiver was observed placing a child in restrictive device as a form of punishment. One child remained in the restrictive device for 63 minutes.”
“The play structure was less more than six feet did not have required surfacing.”
“Two infants were observed in swaddles. This was corrected when CCR staff informed operation staff that swaddles can not be used without a sleep exception form from a health care provider. The swaddles were removed.”
“The operation does not have a current fire inspection. The last fire inspection was conducted on 3/11/25. The operation has submitted a request for a fire inspection as of 3/26/26.”
“The operation did not have a transportation emergency plan available for review.”
“The play structure has missing screw and/or bolt.”
“One child's file did not have release of child information.”
“The policies did not include the promotion of indoor and outdoor physical activity.”
“The parent handbook/operational policies did not include a safe sleep policy for infants 12 months and younger.”
“The policies did not include parent rights.”
“Outlets in the infant room and schoolers area did not have protective coverings and accessible to children under 5 years of age. This was corrected when outlet coverings were placed on the accessible outlets.”
“Eight children files were missing water/swim required information.”
“Two children files were missing health statements and have been in care for over 12 months.”
“The transportation van was not clean and filled with trash, paper, debris, and other clutter on the floor of the van.”
“An admission form for a school age child did not include the school's name and number.”
“Window cleaner was observed in the Pre-K children's restroom and was accessible to children. This was corrected at inspection when the window cleaner was made inaccessible to children by removing the cleaning product from the cabinet.”
“The base board in the Pre-K children's restroom is damaged and needs to be replaced.”
“Based on documentation obtained during the application inspection on 12/27/23, the operation has not received a required fire inspection conducted by the local fire marshal.”
“The outside small playground has a wooden board missing from the fence creating an opening in the fence big enough for a child to go through.”
